I’m a board certified urologist, a practitioner who treats men widely for a variety of different conditions. And Donna Lee here’s our practice manager.

I don’t think it’s just because we said.

It is true. We gave ourselves an award. It was an award I had in junior high for being in the band.

Robert Mendiola is our registered dietician. Welcome today, Robert.

Robert : 

Hey, thank you for having me.

Dr. Mistry: 

You know, throughout the year, we’re going to talk very much in depth about all the different urologic things that we deal with. Kidney stones, prostate cancer, kidney cancer, low testosterone ED. We’re going to talk about them in depth. One of the central things that’s a real focus here is the importance that nutrition and weight loss plays in all of these conditions. Would you agree, Robert?

Robert : 

Sure. Yeah, definitely.

Dr. Mistry: 

And I think that at the beginning of the year, especially as so many men and women make these resolutions to lose weight. I thought I’d bring you in and maybe get your insight into how does nutrition play an important role in urology conditions? And then we’ll talk about what kinds of tools we give patients to help them lose weight better.

Robert : 

So I think that across the board a lot of conditions do get improvement with weight loss, right? So things like low testosterone are going to be very responsive to changes in weight, higher cholesterol, prediabetes, which are really common things that come tied in with these different urologic conditions, right?

Dr. Mistry: 

Including erectile dysfunction.

Robert : 

Absolutely. Yes. Right, right. And so, and you know, just as well, when you see urologic condition, there are going to be a lot of sometimes comorbidities or other medical issues going on there as well. And so being able to kind of offer that whole program, that whole picture of, “Hey, we, you know, what are we taking in and what’s our diet looking like right now?” I think that connecting those dots for people can be really insightful and really help them kind of get their health back on track.

Dr. Mistry: 

A lot of people are surprised, too.

Robert : 

Sure.

Dr. Mistry: 

I mean if you are 350 pounds and five foot 10 and I diagnose you with prostate cancer and you have diabetes, high blood pressure and obstructive sleep apnea, my decision to operate on you is going to be significantly altered…

Robert : 

Right.

Dr. Mistry: 

…as opposed to you being 260 pounds and 5′ 8″ with only one medical condition. It’s a different animal.

Robert : 

Right. Yeah, and I do let patients know that as well. I say, “You know, Dr. Mistry is really supportive of getting our eating right and what does that look like for each person?” That might be a little bit different, but finding what that is, finding how we can get them to stick with it, how they can kind of feel good at the same time. And so part of that is the accountability that I’m able to give them as the health coach here is having these check-ins and saying, “What’s working, what’s not working? How are you feeling and how do we kind of get the ball moving still?”

Dr. Mistry: 

It’s more than just getting, you know, a one time visit on…I mean, let’s just talk about what healthy eating is because we could have a handout for that, right? It’s, it’s about an individual practice, an individual program, depending on your disease process. Whether it be kidney stones or cancer, whether it be something specifically related to low testosterone. Not everybody has to lose weight, but a lot of people do. Food allergy testing, nutrogenomics. What have you learned the most about in working here with our patients?

Robert : 

I’ve learned that people are really curious about nutrition and diet and lifestyle, and there are still so many unknowns. And I think that’s really, kind of a fun part of working in like nutrition and like a science field is there’s so many unknowns still, right? We can find evidence to support almost anything that we want it to support no matter what. Right. We could find a study that says eating candy for breakfast every day is really good for you. Right, right, right. Yeah.

Dr. Mistry: 

Let’s find that one! There’s probably the same fasting study that you can’t eat anything the rest of the day…

Robert : 

That’s right, yeah! But people are very curious, you know, and they are well-informed. And they come in with really good questions? And I’m seeing more and more younger men who are taking charge of their health even when nothing is, you know, in labs, nothing is out of normal limits. Let’s say their weight is normal as well, but they’re really just come in and saying, “Hey, am I doing stuff right? Am I taking care of my body?” And I think that’s really cool because I think that’s really changed. I think men in particular and young men, right? I mean historically people are going to the doctor when they are sick, right? And they’re like, “Oh, I guess I’ll go in,” or people around them are nudging them, like go take care of yourself. Right? So to see younger people doing that that are, you know, maybe in their early 30’s I think is really impressive, and tells me a lot.

Dr. Mistry: 

You know, I think that you hit on such a, such an insightful point. You know, as physicians, we’ve been so jaded by the fact that our patients don’t listen to us every time. I think a lot of the opportunity to educate patients on just basic nutrition has kinda gone away. And now patients really, they really are curious, and you don’t fix everyone. And that’s really not the goal. The goal, I mean, I guess it’s sort of goal to fix everybody, but it’s not, it’s not the metric by which we judge ourselves successful. Just finding people that change their habits into healthier habits, making sure people are aware that when they stick that 7th potato chip in their mouth, that this perhaps isn’t as good for them as they perhaps are thinking.

Robert : 

Right. Right, right.

Dr. Mistry: 

And I love the fact that we have so many patients that are healthier, that are looking to make changes in their health because that’ll keep them healthier. How do you keep people motivated?

: 

We do kind of a number of things. I just kind of do a check in. I kind of start our visits with saying, “Okay, well, how have things been since our last visit?” And then we use whatever accountability tool we’ve kind of started or educated or going…went over and maybe our initial visit that might be something like a food journal we get to review together. I know a lot of people are not very big on tracking food. So I usually tell people, if you give me two to three days of eating track, that’s a really great glimpse into your life. And then we can make some changes based off of that.

Donna Lee: 

It sounds daunting, but it’s so important.

Robert : 

Exactly. Yeah. And you…

Dr. Mistry: 

And some people don’t want to tell you how much alcohol they drink.

Robert : 

Exactly. Yeah. So I tell people, you know, don’t be well behaved on this. Be your real actual 100% self. If you eat a pizza and 12 beers, please put that in the app so we can talk about that.

Donna Lee: 

Whoa, hey! Party at Robert’s house!

Dr. Mistry: 

How did he know what I had for lunch?

Robert : 

And so part of it is that, maybe reviewing some of these tools, some people that are maybe not tech savvy, I try to put them on paper so it’s something they can still feel like they are accountable with and even if they’re not comfortable using their phone like that. And then just kind of navigating some of those speed bumps in the road bumps that come up with a busy life and still trying to be healthy. Right? Yeah.

Dr. Mistry: 

Absolutely. And I think that sometimes, and especially if you’re really motivated at the beginning of the year, that you should really think about doing something that’s very medically managed and we’re very happy to be able to provide ideal protein here. It’s a medically managed program. It’s strict, especially a time of year when you’re okay to be a little strict because you want to see big gains or big losses. And you know, we, I myself have done ideal protein. I did achieve the 5 to 7 pound weight loss a week. Why don’t you just briefly tell people about the IP program and what can they expect?

Robert : 

Okay. So our IP program is basically a medically managed, ketogenic diet. So we do get our body using fat for fuel. It is lower calorie as well, and it is moderate in protein and pretty low in carbs. One of the great things that it does take the guesswork out of “What do I have to be eating? What does that look like?” Which we all have a lot of questions on. And a lot of people don’t have time to answer all those questions before they start changing their health for the better. And so it’s a really great way to have some accountability, set up some weekly appointments with the health coach, and navigate some of the program and say, “Okay, well how’s our weight trending, and have we hit our goal, and what’s working?”

Dr. Mistry: 

And if you’re a man out there and you’re looking to lose about 40 to 50 pounds in 12 weeks, I think it’s definitely a very achievable goal. Would you agree?

Robert : 

Yeah, yeah, I do.

Dr. Mistry: 

And what can people expect to pay about a week for that program?

Robert : 

So it would be $300 at the initial visit. That kind of starts you off with supplements, a week’s worth of food kit, kind of the whole deal. We go over in the office and the program and then after that it would be your week’s worth of food. So about $90 a week after that.

Donna Lee: 

Wow. That’s really inexpensive. I can order that today.

Robert : 

I think about how much time, how much you might spend eating out or things like that. It can, it can balance out really well.

Dr. Mistry:
